Oath

The Township of Lucan Biddulph provides Commissioner of Oaths services. We commission documents like Declaration of Vehicle Transfers and general affidavits. To have something commissioned, everyone who needs to sign the document must be present and have valid photo ID.

Appointments are strongly recommended to ensure a Commissioner of Oaths is available. Please contact the office at 519-227-4491 in advance or submit a request for commissioner of oath services using our citizen portal request located on our website.

Commissioning services are provided Monday to Friday, 8:30 a.m. to 4:30 p.m. at the Municipal Office, 270 Main Street, Lucan. The fee for this service is $10 for Lucan Biddulph residents or business owners and $20 for non-residents.

The Commissioner is not responsible for the content of the affidavit - That is the responsibility of the person whose signature is being commissioned. You must understand the details of what you are signing and their correctness.

Note it is at the exclusive discretion of the individual Commissioner of Oaths whether or not they choose to sign the document. A Commissioner of Oath is not a Notary Public and cannot notarize documents.  The Township does not have a Notary Public on staff.

 

Documents we can Commission:

  1. Sworn statement for the transfer of a family gift of a used motor vehicle in the province of Ontario
  2. Domestic and foreign pension documents (proof of life)
  3. Affidavit of unregistered vehicle (ATV’s, motorcycles, snowmobiles, trailers, etc.)
  4. Consent to Travel
  5. declaration regarding residency (with accompanying documents)
  6. Legal name change forms
  7. School Immunization Forms
  8. Statutory Declaration Form (Contractor CCDC 9A)
  9. Statutory Declaration (Form 23) – registration error for Birth, Marriage, Death
  10. Statutory Declaration for OSAP (Common Law)

 

Documents we cannot Commission:

  1. Wills and Powers of Attorney (including Living Wills, Codicils to Wills and Statements of Assets of a Deceased Person)
  2. Estate Settlement Documents
  3. Invitation Letters
  4. Divorce or Separation Documents
  5. Custody Documents
  6. Insurance Documents
  7. Financial Documents
  8. Real Estate Documents
  9. Other Personal-related Documents
